How can we help you?

The Patient-Centered Medical Home, (PCMH) is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family.

Evidence Based Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner. (NCQA).

Provide you with more personalized care
                       

  • Because your physician and office team knows you and your health history.

Be an active participant with your health care team.

  • Call and ask questions, bring a list of concerns and questions to discuss with doctor, nurse or office staff.

Provide a safe environment

  • To talk about confidential concerns.

Provide us with a list of all medications

  • Including supplements and treatments you buy in the store
  • or obtain from others so we can help prevent interactions.

Provide responses

  • To your questions and concerns.

 

Call us before you go to the emergency room

  • Or let us know as soon as possible that you have been to the emergency room,
  • Or urgent care.

Provide an explanation

  • Of services offered within the practice, for example why we are taking blood today?

 

When you go for care elsewhere,

  • For example (drugstore for flu shot) make sure to tell them who your doctor is and authorize them to send the information to us.

Guide you through the complex health care system

  • To help you get the care you need from our office, another physician office, hospital or other services in the health care system.

Be informed

  • About and involved in your healthcare decisions.
  • Ask us for information about your condition or treatment.

Coordinate your care

  • With you and your family within our office, among specialists, hospital and other health care services.
